PQRI as it relates to our Physical Therapy Evaluation Forms... 

PQRI - Physician'sQuality Reporting Initiative is a quality reporting tool used for Medicare patients. 

"PQRI establishes a financial incentive for eligible professionals, including physical therapists, to participate in a volusntary quality reporting program.  PT's who successfully report on at least three quality measures on claims for dates of service from January 1 to December 31, 2008 may earn a bonus payment of 1/5% of total allowed charges for covered Medicare Physician Fee Schedule services." from www.APTA.org

For 2008, PT's are allowed to collect data in the following 8 categories:

1.  Documentation & verification of medications
2.  Pain assessment.
3.  Patient co-development of plan of care.
4.  Screening for future fall risk.
5.  Universal weight screen
6.  Adoption / use of health IT.
7.  Diabetic foot / ankle care: footwear eval.
8.  Diabetic foot / ankle care: neurological eval.

PT's are only allowed to file with CMS on 3 of the 8; the PTEFs (Physical Therapy Evaluation Forms.com) allow for screening of 1-5

This is an excellent tool in an office which has a high volume of Medicare patients.
